Hm, well, I think John's OOC-ness can be blamed on Moffat - he did that in "Doctor Who" as well, having moments that seemed inconsistent with the character arcs. *g*
And everything else about that last episode was just made of awesome. Stunning, tense, funny - just absolutely great! Marvelous! Magnificent! Etc. :)
Moriarty is totally OTT, but I like that he's not some old professor, but really about Sherlock's age and teasing and hilarious. XD
Concerning the cliffhanger - I can only repeat myself and say, go read likeahurricane7's fanfics, especially "5 times Dr. John Watson surprised Sherlock Holmes" (link to the fifth time, because that's exactly the ending of the season and going on from there). :)
